  As for when Harry holds a trembling/frightened Hermione, I'd guess 
  that would be the point in the book when Harry and Hermione are 
  beside the lake (Harry ran off to save Serious from the dementors, 
  with Hermione following him) and the dementor's are beginning to 
  surround them.  Of course, it could be just after Ron was grabbed by 
  Dog!Serious, but I can't see them just standing there instead of 
  running immediately after Ron.

  I agree!  They look scared for themselves, like something is slowly closing in on them and surrounding them.  They don't look concerned or frightened for someone else or like they are trying to avoid the quickly swinging whomping willow or dangerous werewolf.  In my mind, it must be the dementors.  BTW, they must be real tall, as Harry is looking up quite high.
