The Quaker schools around here, and some other private schools too, 
use the title-with-first-name system for the children to address the 
teachers.  My sister did her student teaching years ago at a Quaker 
school, where she was known as Miss Elaine.  When my daughter went to 
a private kindergarten, her teachers were Miss Yvonne and Miss 
Towanda.  In the public schools, some teachers lately seem to be 
using this system if they feel their surnames are somewhat cumbersome 
for small children to pronounce, thus my son has had teachers that he 
has called Mr. Michael (very long Greek name) and Miss Lisa (slightly 
awkward German name).  Another solution some teachers find for this 
problem is to use the first initial of their last name, such as the 
music teacher at my kids' school who goes by "Mr. C."
