I think you see that term used mostly on the continent.  In Holland I would always ask for the WC (pronounced there as Vay Say) or look for a WC sign.  Here in England, I look for toilet or loo.  Other differences I've found?  Always carry a variety of change on you when you're in Holland as you'll need it for the WC.  While most attendants are helpful (which is where you'll find the clean toilets), there are those who will tell you they don't have change so you end up paying more or will insist you have to pay twice if you are taking your child to the WC.  
 
I remember on my first trip to Paris I used a pubic toilet.  I had to pay to use the toilet and then had to pay for toilet paper as well.  I learned to carry packets of tissues on me at all times.  LOL
