Removing an unauthorized HP vid upload?

Has anyone tried to remove a [song]vid from YouTube? Success rate?
The rest of the message is blah-blah-blah, so can be skipped. Above
question is my priority.
I did not give permission for this vid to be uploaded:

While the person who put it there meant so in a nice fashion, I don't
want the vid there. Offhand, only one site has ever been given
permission to host my vids elsewhere. My option is to contact Faychii
but YouTube requires I create an account with them. Ugh, but I'll do
it if necessary.
As long as I have a good working website (site's been around for eight
years and I just renewed the domain for another four years) and files
are free to download for everybody and I don't mind direct links
(please don't stream the file), I don't want to see my vids elsewhere.
There is no reason for them to be uploaded elsewhere. If my website
dies or I die and can't pay the hosting bills, *then* there's a
reason.

Uh, I'm the vidder of copyrighted video and music... and I'm not the
copyright holder. Good combination. {g}
Other than Veritaserum, if there are bunniqula vids not hosted at
archive.nu, they're not authorized uploads. I'm into making revisions
because I'm a middle-range vidder and I always *require* improvements,
so I'd rather not see multiple versions floating around. If I was
given account access to alter my vids at any time and the webhost is
solid and advertisement-free, then I'd consider other hosting
options... That day has not arrived.
