Dame Margaret or Dame Maggie - never Dame Smith (uttely pointless and pedantic but they 
made me learn this stuff at school!).

The wheelchair is a prop from the film, and, yes, it's related to her recovery from tha attack - 
this came up on another forum a few days ago.
