A bit of both, really. Harry certainly isn't on the periphery of Sirius'
thinking!
Take this bit from PoA, in which Sirius, having described his time in
Azkaban transforming into a dog to trick the Dementors into thinking
he'd lost his mind, says:
'But then I saw Peter in that picture ... I realised he was at Hogwarts
with Harry ... perfectly positioned to act ...'
He goes on to suggest that Pettigrew was waiting to deliver 'the last
Potter' to Voldemort. 'If he gave them Harry, who'd dare say he'd betrayed
Lord Voldemort?'
He realises he is the only one who can do something, the only one
who can track down Pettigrew and stop him from delivering Harry to
Voldemort. That's why he breaks out of Azkaban.
