As someone who has written a novel, I do, too.  I speak from
experience.  I remember when I saw my book cover flat for the first time
(for Emerald House Rising), I was complaining bitterly because they put
a moon on it, and I said, "There is no moon in that world, I made sure
of it; so why is it on the cover?"  And then I re-read the damn thing,
and there was a reference to the rising moon, in the first chapter.  And
I had completely forgotten I'd stuck it in.  Boy, was I embarrassed.

Who sometimes loses things, but rarely anything as big as a moon,
although it does happen upon occasion.
