Who uses the name?

So far, we know that strong wizards (Dumbledore, Sirius, Harry and 
Lupin) say Voldemort.  We know that Crouch/Moody, McGonnagle, the 
Weasleys do not.  I don't remember about Hermione or Snape, and I 
think Crouch Sr. does.

But in PoA, Wormtail refuses to use the name in the Shrieking Shack, 
then uses it, then doesn't.  In GoF, I think Wormtail never uses the 
name.  Is this a mistake, or does it show some sort of divided 
loyalty of Wormtail?
