The actual quote is "I saw you(Lupin) running along this passageway and out of site".This would imply that it MUST include the grounds, as "this passage" only starts at the Whomping Willow, which is in the school grounds. 
And as final proof that it shows the grounds as well, when Fred and George first give Harry the map, they show him the seven passages out of the school. The four that Filch knows, the one by the WW, the caved in one and the one to Honey dukes and they all "run of the page". It says (PG 144 UK) "It was a map showing every detail of the Hogwarts castle and Grounds" which answers your question about what the Map shows.
