If he was that ill, I don't think they'd have dragged him out of 
Madame Pomfrey's care to the dinner at the end of GoF ~ Harry sees 
Moody up at the head table, looking unwell but definitely present.  
Also, considering Moody's paranoia, would he want to be shipped off 
to St. Mungo's, or would he want to stay at Hogwarts, with such 
powerful wizards as Dumbledore, McGonnegal, & Snape, and doing his 
part for the 'cause' by teaching the innocent little kiddies all 
there is to know about defense against the dark arts?

On the other hand, Dumbledore had initially only convinced Moody to 
teach at Hogwarts for the one year, due to the Triwizard Tournament ~ 
right?  (Don't have the book on hand, and my memory is known to fail 
me, so I could be wrong.)  So perhaps he *won't* stay on, as it 
wasn't in his plan in the first place.

The reason I myself have suspected that he'd stay on to teach DADA is 
that JKR has said that Harry would be getting to know the real Moody 
soon.  It seemed logical, if not necessary nor certain, that one way 
Harry would have such a chance would be if Moody stayed on to take 
the DADA spot.
