I think what's most likely is that they keep a small journal of 
everytime they award or take away points with the incidents involved, 
and periodically give the journals to Dumbledor to be tallied. That 
way, there are checks as to whether or not the whole thing is legit.

Your theory has a hole: Gryfindor gets -150 points over the Norbert 
incident (PS) at 2 or 3 in the morning, and by 9 all the school knows 
about it and is already reflected in the Gryfindor hour-glass. Even 
though it'd a special circunstance, I don't imagine McGonagall waking 
Dumbledore just for that reason. A much simpler explanation is that, 
for the hour-glass enchantment to work, the student involved must hear 
the words from the teacher. It amkes sense, since it's supposed to be a 
way of (un)encouragement: if the sinner doesn't know he's sinned, he 
won't learn.
