hmmm...
I always call a teacher Ms. or Sir until I find out their name, then it would 
be, for example, Miss Ashworth and Mrs Smee and Mr Morgan, but that's just at 
my school.

Some of my Friends go to a comprehensive and have to call ALL their teachers 
Sir and Madam regardless of whether they know their names or not...

The only problem with the Miss Mrs system is if you get it wrong they 
completely wig at you...not fun! So generally, unless you're 100% sure of 
which it is, it's generally (eg) Ms. Beale...ie, you blur the words so you 
can't really tell what it is you're saying!
