You're correct, she isn't referred to as the "Gray Lady" in any of the 
books. However, Rowling herself has called her that in a couple of 
interviews. I also remember watching Rowling on "60 Minutes" a US 
television news program. There was one brief scene where they showed 
some of her notes. You could read that it was a list of the House 
Ghosts and Ravenclaw's was listed as the "Grey Lady".

About Harry's method to kill Black in PoA, I think Harry is so enraged 
that he contemplated hurting Sirius using non-magical means. Remember, 
Harry did punch Sirius a couple of times during that altercation. Also 
remember that Harry was raised in  the Muggle world, so resorting to 
magic might not be the first thing he thinks about.
