It's good to remember that there are good reasons why the
*characters* made the mistakes and not the author. Until Lupin looks
at the map, he has no idea of what is going on. Until Snape uncovers
himself, they have no idea that they have to deal with him. Until
Harry stops them from killing little Petey, that's exactly what they
intend to do.
Granted, Sirius was supposed to be just about the top of his class,
and Remus is probably no slouch, but even the smartest, wiliest, and
cleverest out there won't think of the obvious answer when things are
moving very quickly and you're faced with a situation for which you
are totally unprepared (remember Hermione wailing about not having
wood available to start a fire to chase away the devil's snare?)
When that happens, you make dumb decisions that -- in hindsight --
seem so obvious, but at the time it's just what you come up with.
