Ah! But remember what Black looked like? To quote PoA (chapter 11, page 212): 


"If he hadn't know it was the same person, he [Harry] would never have 
guessed that it was Black in this old photograph. His face wasn't sunken and 
waxy, but handsome, full of laughter." 

That explains why she may not've recognized him physically. I can't, however, 
explain why she didn't recognize the name. ^^;
