I don't think the Dementors actually forced him to change, at least 
not intentionally.  I think the cumulative effect of 100 Dementors 
closing in on him was just so strong that even being in dog form was 
no longer enough protection and Sirius was just overwhelmed.  He may 
have even temporarily forgotten that he was an Animagus (becoming 
one was a happy memory, right?)
