Sirius died because he was bored, feeling useless, and he was too head-strong. No other reasons. MHO.

the Dark Lord would probably try to falsely lure him
to the Ministry of Mysteries for the prophecy. If he
had been honest with Harry a few months earlier,
Sirius would be alive. 

4. Sirius contributed to his own demise because of his
feelings of worthlessness, which I might add aren't
totally his fault.
