What if he stummbled upon the Chamber by accident?  

I don't know...I can just picture Tom having to use the bathroom really bad, then going into the girl's room because no one was there and it was close, then noticing the snake carving on the facet....etc..etc...and the rest is history.  

As for him knowing he was the heir of Slytherin. Well, my guess is that he may have had a strong interest in his mother. After all, he never knew her. I could see him tracing his family history and discovering Salazar Slytherin at the end.
