Could it be that Snape never made the connection until he placed 
that memory in the Pensieve? That perhaps, he did hear Sirius call 
James "Prongs", but (totally projecting here) wasn't sure he heard 
it correctly at the time, and then that stood out more upon 
reflection? I thought that was what the Pensieve was ultimately for: 
to help see patterns in thoughts in order to reach a conclusion.
