I don't know about Voldemort, but The Leaky Cauldron cites a review 
of Dale's recordings that suggests it's the guidance in GOF on how 
to pronounce Hermione that is the cause.

I think the two pronunciations are just a matter of accent, myself.  
Neither is wrong, IMO.  Her-mah-nee is how Justin Finch-Fletchley 
might pronounce it.
