Nope. He calls Pettigrew Wormtail. Could be that just like anyone 
else he starts using more personal names once he's worked closely 
with someone. Maybe Malfoy and Bellatrix were particularly close 
associates of Voldemort before his fall.

Another thing would simply be that there are at least 3 Lestranges 
among the Death Eaters. Calling them all by their common last name 
would prove unpractical.
