I don't think anyone person can be assigned sole blame for the death, 
except Bellatrix, since she did the actually killing, or at least 
used the spell that shoved Sirius through the veil.

Having said that, certainly the actions of the other players 
involved - Harry, DD, the other OoPs, Kreacher, Sirius - all played a 
part.  Had any of them acted differently, the end result may have 
been different. Had Harry used the mirror or continued with his 
Occlumency lessons, had Sirius decided to stay home and sit on his 
hands, had DD taken Harry aside earlier to give him more 
explanations, had the OoPs joined forces to magically lock Sirius up 
in the house, even if the pairings of people dueling at the Ministry 
had been different, the outcome may have been different. With so many 
people involved, the number of variables climbs quickly.

But, I do think that this will weigh on Harry's mind, and I wouldn't 
be at all surprised if he feels a measure of guilt, as least for a 
while.
