Well why shouldn't they? Look at what you said in your own post *the rest of
the order*. Sirius is part of the order, why would they take everyone else
and leave him behind. The rescue mission is quite possibly the *least*
reckless thing Sirius has done since we first encountered him back in PoA.
He's not running off on his own to do something he's part of an organized
rescue mission to protect his Godson. The reason they've been making him
stay at home for the rest of the book is because he's a wanted man and might
get arrested if he was out in the WW not because they thought he was likely
to get killed. Going on the mission did run the risk of him being arrested,
but since none of the Order should have been there anyway they were probably
all running that risk and I think Sirius would have considered his freedom
insignificant when compared to the life of his Godson. Sirius was in no more
danger of getting killed on the mission than anyone else. It could just have
easily been Tonks, or Molly or one of the DA that got shoved through that
veil.

And as for who's fault the death was I'm sure all our heroes will be feeling
guilty but while their decisions contributed to Sirius being in a position
where he could be killed the only person truly responsible for his death is
Bellatrix (althoug since she's acting on Voldemort's orders I suppose we can
blame him too). If Dumbledore had been a little more forthcoming with Harry
Harry *might* not have run off to the Ministry (but bear in mind he already
knew that his visions *could* be true and I'm not sure he would have been
willing to risk being right and doing nothing); if Severus had been able to
overcome his temper and hatred of all things Potter and carried on Harry's
lessons while punishing him in some other way Voldemort *might* not have
been able to get into Harry's mind, but there's no guarantee of that; If
harry had trusted that the mesasge had got through and not gone off to help
then Sirius wouldn't have had to rescue him, but Voldemort would no doubt
have tried something else and someone else might have died instead. While
Dumbledore makes a big thing about it being our choices that are important
no one can foresee all the consequences of their actions. I think the Veil
will be important at a later date - so who knows maybe Bellatrix' decision
to kill her cousin will somehow lead to Voldemort's downfall.

Sirius' death is no more Harry's fault than James' death is Sirius', but no
doubt he will feel jkust as guilty as his godfather did - not because he is
responsible but because he is essentially a good person who cares very
deeply for those around him and would do anything to protect them.
