I think "O" for outstanding outranks "E" for exceeds expectations.  

Just a thought, technically, surely, if say your teacher expected to 
you to get a "D", and you did better than that you would get an "E" 
which would take you above the "A" that would normally happen if you 
outdid yourself and upped your grade?
