Barty Crouch Sr referred to LV as the Dark Lord when he tried to warn
Harry before he was killed by his son. Trelawny referred to him as the
Dark Lord ONLY when she was doing a 'real prediction', which she never
remembers afterwards nor seems to know what she's doing when she does
give 'real' prophecies.
Note that Dobby referred to LV as the Dark Lord more then once. The
person who wrote Harry's valentine (Ginny?) called LV the Dark Lord in
the poem.
I think Harry is generalizing way too much when he says ONLY the Death
Eaters call LV the Dark Lord. Not surprising as most people that age
can only see in clear cut black and white. Like Sirius told him, the
world is not neatly divided into Death Eaters and Non-Death Eaters...
