Perfects & Head Boy/Girls (was: Prediction)

Sorry to change the subject, but I'm wondering about the Prefect and
Head Boy/Girl system in the school.

So, how does that work?

Prefects are picked in 5th year, so do the 5th year Prefects remain
Prefects in their 6th and 7th years? That would make a total of 6
Prefects for each house; 2 in 5th, 2 in 6th, and 2 more in 7th year. 

Or are the new 5th year Prefect the only Prefects in a given house? 

I think Percy is Prefect every year until he becomes Head Boy, so I
think the first option is probably the right one. At any given time,
there are 6 Prefects per house.

Now Head Boy/Girl; is that a house honor or a school honor? Six
Prefects per house and one Head Boy and Head Girl for the entire
school, or one Head Boy and Head Girl for each house.

If it is a Head Boy&Girl per house, then are they picked from the
Prefects or from the student body in general. If they are picked from
the Prefects, do they replace two of the Prefects leaving 4 Prefects
and a Head Boy&Girl, or is it still 6 Prefects plus a Head Boy&Girl?

While I know the method of picking them varies from school to school,
am I right in assuming at Hogwarts, teacher pick outstanding students
rather than the students picking the most popular?

Just a few thoughts.
