Well, I'm thinking nothing  more than a very touching and pathetic (as in "filled with 
pathos") moment.  What I see her communicating is that even in her madness she still 
has some vestigial maternal sense that makes her want to reach out and be kind; and 
where is she going to find anything else to give him in the ward?  After all, it's 
something of hers, not just a piece of silverware.  That Neville keeps it makes my  
heart ache for him and love him all the more.
