They are the initials of the prophet and the person that recorded the 
prophecy - i.e. Trelawney and Dumbledore.

It was an amphiteatre-like room with a arch in the centre (which like 
all amphiteatres is lower than the stands). Under the arch there is the 
black veil, which is a symbol of death. Sirius falls through it and 
naturally dies, since he "crosses the veil". The Department of 
Mysteries studies the big mysteries - death, in this case.

Sirius must have been standing in a grade under Bellatrix, thus when he 
is hit by her spell he's pushed thorugh the veil.
