Were we ever specifically told that Snape was in Slytherin House when 
he was at school?

I know he's the Head of Slytherin now;  but I don't remember reading 
that it was his school house.

And who heads Ravenclaw?  Flitwick?
Also- were Lily and James both in Gryffindor?
