"The first one we finally found out about, was recorded in the DoM.
What about her second prophecy in PoA? She predicted Wormtail's return
to Voldemort. Was that never recorded? Why? How is one prediction
recorded, yet not the other? Any thoughts/ideas?"
Well, we're not really sure how prophecies are recorded, if they're
done while they're happening or after the fact (possibly by using a
Pensieve?). But in PoA, Trelawney's only witness to her second
prediction was Harry (remember, she made it right after his exam with
her, so they were alone together I presume). He had no way of
recording it at the time or even the knowledge that such things could
be recorded. That's probably the reason it wasn't recorded. If it's
possible for prophecies to be recorded "after the fact" in some way,
then either Trelawney or Harry would have had to have done it. But
remember Trelawney didn't even remember the prediction she had made
(she didn't know what Harry was talking about when he asked her about
it, she thought she had fainted or something), and Harry certainly had
no idea how to record such a thing, even after the fact (if that's
even possible).
I'm sure if the prediction had been made to Dumbledore, it probably
would have been recorded. But instead it was made to a teenager who
had no idea that recording it was even possible.
Oh, I'm new, by the way. Nice list you've got going here.
