Head Boy and Girl, I am supposing, is awarded based almost entirely
on academic performance, whereas, prefects are students who have
displayed responsibility and /or qualities of leadership.
Though this is a canonless post I am sure if you dig through the
books a little you'll see that this is as close to implied meaning as
you can get.
Lily and James were, indeed, Head Boy and Girl of their year.
