Yes, they are sisters. I'll quote bits of the entire passage that
shows the connection (US hardback, pages 113-114):

By these quotes, I doubt it's possible that Narcissa isn't really a
Black sister/cousin. If the Black family burns names off for
being "black sheep" (sorry, no pun intended), then I can't imagine
that they would include a foster/adopted daughter. They're too
paranoid about the pure-blood thing, plus they're OBSESSED with the
family line being pure.
