Well, it is an interesting theory, but I have a few problems with 
it. If it was premeditated and not accidental, like your final 
suggestion, what possible motive would Lupin have? And another 
problem-that's just too cruel. JKR has already shown betrayal among 
friends, why would she do it again here with Lupin killing Sirius?

I don't agree with your theory that it was Lupin, but I do agree it 
is not necessarily Bellatrix. I was just looking back at OotP and 
she says in the MoM Atrium "What did you come after me for, then? I 
thought you were here to avenge my dear cousin." Which does lend 
credence to the theory that it was not necessarily Bellatrix.

Although...I feel a fanfic being born in my mind based on this 
theory...
