I just find it hard to believe that someone as composed as Snape 
would clutch his arm, thus exposing for certain his dark past to 
everyone in the room, if the statement were simply in reference to 
the mark. I believe there must have been more to it, because Snape 
would never be so indiscreet.
 
Now Carol:
I agree with Bonny. Snape seems to have actually been in pain. He
would never have given himself away like that at a mere allusion to a
literal or figurative stain. Somehow Crouch!Moody seems to have
activated the Dark Mark, as if, through him, Voldemort sensed
disloyalty in Snape and was punishing him. But Snape, who at that time
only knew that "Moody" hated him for being an ex-DE, must have found
some other explanation for the pain. Almost certainly Crouch!Moody
*could* see it, just as he saw Harry's mismatched Dobby socks at the
Yule Ball. Snape also apparently feels pain at the mention of
Voldemort's name, which IMO is the reason he refers to LV as the Dark
Lord and doesn't want Harry to say the name.

Normally Snape would have sensed that Crouch!Moody was not who he
appeared to be, but since he already regarded him as an enemy and
avoided looking into his eyes, he missed clues that normally would not
have escaped him. In this instance, he was concerned at firt about his
office having been broken into and then with his certainty that Harry
was on the stairs in his invisibility cloak. In that state of mind, I
don't think he questioned the pain in his arm. It's possible, though,
that he reported the incident to Dumbledore, for whom it was one more
suspicious detail that eventually led him to conclude, seemingly from
too little evidence, that "Moody" was Barty Crouch.
