I don't strongly believe in the Double Agent Snape theory.  But if it's 
true, then the reason Snape still calls Voldemort "Dark Lord" and not by his 
name is because he does not want to get out of the habit of doing so -- as 
his first mistake of that kind at a Death Eater meeting (with or without 
Voldemort present) might be his last.
