About the first question, I've stated that I believe that a long chain of people - the first casting the charm, the last being the secret-keeper, and all but the last believing that the secret-keeper is the next, is good enough.

About the second, I think Voldemort might not have decided to act immediately. I think that when Voldemort did decide to act, he told Snape, and that was why Snape decided to join Dumbledore.
