Umbridge (I know she doesn't really count) calls Draco by his first 
name then either calls him Malfoy or Mr. Malfoy a few moments later 
when he pushes to go into the forest with her and she gets mad.

Lupin calls several of the Gryffindors by first names (chapter 7 
PoA) while he is a teacher.

I think (but it may be movie contamination here) that Lockhart did 
as well.

McGonagall refers to James and Lily after they are killed.
