I guess each could be correct.  Perhaps Mr. Ollivander did sell the wand 
to Tom and then later was told that Tom is now LV.  If he is a great wand 
maker/seller he might have wondered what type/size wand the Dark Lord had 
and where he got it. Perhaps someone told him where LV came from.  I'm sure someone knew who LV was and where he came from.  They have always said LV 
was afraid of DD and I'm sure DD know as much about LV as is possible.

On the other hand, I'm guessing LV didnt' just come out killing people.  So 
he very well could have walked in to Ollivanders wand shop in the early days 
and buy a wand under his LV name.  I'm thinking in the beginning the DE were 
a type of underground society.  Secret meeting, secret hand shakes and all.  The name LV at first might have meant nothing, but after the killings and uprising the name meant much more.

I'm leaning to the first.  Tom had the wand and Mr. Ollivander was asking 
around trying to find out where he got his wand. Perhaps he was scared that 
something he created was being used for such evil. Imagine working so hard on 
something as beautiful as a wand and finding out it was being used to kill 
people......
