I think the only significance of the Christmas presents is that
exactly one year before Hermione is very upset with Ron for not
realizing that she's a girl. Ron's present is a signal (which he
probably doesn't realize he's sending) that he remembers that
incident, and that he is showing her that he does know *now* that
she's a girl. It doesn't really matter how nice the perfume is,
whether or not Hermione actually "likes" perfume, or what kind of
gift Harry gets her.
I'd be willing to bet that the perfume didn't smell wonderful (hence
the "unusual" comment) because Ron wouldn't know much at all about
selecting scents and also because he doesn't have much money.
However, I don't think Hermione has missed the real significance of
the gift of perfume although I think that the significance is purely
subconsious on Ron's part. After all, he doesn't seem anxious to
see her reaction to it, or even be interested in analyzing her
response. If he were aware that he fancies her, surely he'd me more
concerned about that.
