Using the Time-turner, Harry and Hermione travelled back in time three
hours. After saving Buckbeak from an execution they were waiting for
everyone involved to emerge from the Shrieking Shack. They ran over to the
lake, opposite side from where their past selves (or rather Harry's past
self and Sirius) were. Harry kept on waiting for his dad to show up somehow,
and then realized that it wasn't his dad he had seen, but his own
time-travelling self. He produced the Patronus, knowing his could this time
because he had already seen himself do it several hours earlier.

It's a bit of a complicated scene, just keep in mind there are two Harries
and two Hermionies.
