I asked the same question some time ago. I can't find my original

I think we agreed that the "defiances" were not necessarily physical
conflicts (duels). In fact, they were almost certainly defiances of
Voldemort's will (or his supposed authority) rather than some sort of
defeat, as some readers have supposed. Beyond that, I don't think we
reached any consensus.
