It's simple really. Regulus took Kreacher with him and had him drink
the potion. Dumbledore tells Harry (when they get into the boat) that
Voldemort would not consider a 16-year-old a threat and that is why
they were able to get past the enchantments.
Voldemort would never even consider a house-elf a threat, and a house-
elf could be forced to do as his master commands.
This idea came to me when I remembered the scene in HBP where
Slughorn made the house-elves taste-test ingredients for poisoning.
