We know that Snape brought the potion to him at least once, so I've 
always figured that maybe Snape was on his way up with the potion 
when he sees Lupin run out and follows him.  If so, it wouldn't be a 
case of Lupin forgetting, just that Snape and he hadn't met up yet to 
give and get the medicine.
