Yes, she did, but as I clarified in another post, you were
specifically referring to attempts by Harry's *teachers.* At the time
Umbridge set the dementors on Harry, she was in no way a teacher of
anyone; she was a MoM official.
During her tenure as Harry's *teacher,* she did unspeakably sadistic
things to him, but did not attempt to kill him.

Umbridge.
Actually, except for the fact that Werewolf!Lupin was in the forest
at the same time as Harry, Harry was in no danger from Lupin at any
point that I can remember. There was no confrontation. I might be
wrong, please correct me and provide a canon reference.
