Would Kreacher obey Harry if Regulus was still alive?  Regulus living
would be a male in the direct line of the Black family, much  closer than his 
-- please correct me if I've mixed up the genealogy --  female
cousin Bella.

I agree. That must mean he couldn't have used Kreacher at all, then, since  
Sirius was alive until very recently, and would have had control over Kreacher  
the whole time.
