Now me (Nickel):
"And a new portrait had joined the ranks of the dead headmasters and 
headmistresses of Hogwarts ... " (HBP, p584 Australian / UK edition)

So, I guess that means that 'yes' the headmaster/headmistress must 
be dead before their portrait is displayed.
