Well, when McGonagall transforms, she's certainly keeping her robes. 
We've known this for ages:

"He turned to smile at the tabby, but it had gone. Instead he was 
smiling at a rather severe-looking woman who was wearing square 
glasses exactly the shape of markings the cathad had around its eyes. 
She, too, was wearing a cloak, an emerald one." .. (PS, Paperback, 
page. 13)

So, I'd say that animagus keeps the wand where it is - pocket, sleeve 
or holster... (BTW, where did McGonagall keep that MAP she was 
reading?)

As for Sirius, his wand in OOP was purchased by Dumbledore (As strict 
as Ollivander is, well- Dumbledore is one of the very few who can 
give him Phoenix-feathers and Ollivander needs them, so...); Sirius' 
own, original wand was not with him but in the saddle bag of his 
motor-cycle when he ended up in Azkaban or he had two to begin with, 
other one was kept safe somewhere and he got it; the wand was his 
mother's; it was a Weasley-prank-wand as Sirius never did cast any 
spells as I recall; he bought/made one while in south where the 
British Ministry of Magic has no authority; something else...
