Madam Pomfrey (Poppy Pomfrey),  Madam Pince (Irma  Pince), Madam Hooch 
(Rolanda Hooch - HP Lexicon, from trading card game),  Madam Edgecombe, 
Madam Malkin, and Madam Marsh, all obviously last  names.  So why would 
Madam Rosmerta be different and be a first  name?

I don'r have PoA to hand, but doesn't Fudge refer to her as just "Rosmerta"  
in the scene at the Three Broomsticks?  If it's her last name, I'd  think that 
would be INEXCUSABLY rude of him.
 
BTW, with regard to the name:
