Kreacher contacted a mistress, Narcissa, whose interests he 
knew were in conflict with his master's, and did not inform Sirius of
the fact. He took advantage of Sirius's confidence that the Elf was
not serving anyone else, and that constitutes a betrayal. That 
Sirius's confidence was misplaced is irrelevant -- confidence in
a traitor always is.
