Patronus=Animagus? / Did DD Know that Harry Would Need Glasses?

Can anyone point to something definitive in canon stating that one's 
patronus and animagus forms are the same? I can't recall it, and 
need to know....  Thanks!

Perhaps it has to do with attention to detail and not visual acuity? 
Those are two entirely different qualities.
